Monthly Hoist, Come-a-long, Chain & Sling Inspections - Mt. Hope

General Information 

Hoist/ Come-A-Long Inspection Criteria
Always Oil and Grease during the first week of each month.

Hoist: Inspect structural members of boom or supports for cracks rust or deterioration. Run Chains and Ropes out to inspect sheaves and hoist material condition. Inspect hooks for elongation or twist. Check hoses, wires and motors.

Come-a-long: Inspect function of hoist. Inspect hooks for elongation or twist, run chain/rope out to inspect sheaves and rope condition. Chain/rope should not be twisted or distorted in any way. If they are the Hoist needs to be "Tagged Out"


NOTE: If there are any items that need to be repaired, the Hoist must be "Tagged Out" until maintenance is completed.

1. The Hoist / Come-a-long is operational
2. Operational Condition: No unusual sounds or noise due to binding or bearing squeal.
3. Chain / Rope Condition: Inspect for kinks, gouges, broken strands, stretching, bent or        fraying. No nicks, gouges, or welding splatter. 
4.Hook Condition: Inspect for elongation, bending, cracks.
5. Safety Latch / Clips Condition

7. Controller Condition
8. Lever Condition: Inspect for bends, cracks, or other damage
9. 
Sheave/ Pulley Condition: Inspect for proper reeving

Chain Inspection Criteria

1.     If chains are used for lifting, they must be visually inspected before each use and monthly thereafter with a documented inspection record which includes the date of inspection and an identity of the chain that was inspected. 

 

2.    Any chain that doesn’t pass inspection will either be repaired & load tested to the manufacturer's specifications, or depending on the amount of wear or damage, be discarded & replaced.

 

3.    A designated & qualified employee will perform the chain repairs.

 

4.    All chains will be given a visual inspection prior to each use. This inspection shall consist of looking for excessive wear, defective links or welds, noticeable stretch, and any other obvious problems.

 

5.    If any problems are found during the visual inspection, the employee will notify a supervisor of the problems so an assessment can be made as to whether it is safe to use.

 

6.   In addition to the above, all the chains shall be kept clean and stored in a fashion so as not to cause tangles and binding. The chain hooks or rings shall be secured to the chain links by the approved hardware. Bolts, pins or other makeshift attachments are not to be used. Also bolting, wiring or knotting may not shorten chains

 

7.    Record any items that show signs of possible failure and are in need of maintenance. Said chain shall be tagged and taken out of service immediately, until repairs are completed by trained personnel.

   

           Please Review The Following for Chains:

  

A.       Excessive stretch of the entire chain assembly.

 

B.       Excessive stretch of any individual link.

 

C.       Damage to any hook, ring or other chain attachment.

 

D.       Any kinked or bent links or attachments.

 

F.       Bolts, pins or any other unacceptable attachments.

NOTE: If there are any items that need to be repaired, the chain must be "Tagged Out" until maintenance is completed.

Sling Inspection Criteria

1.     When slings are used for lifting, they must be visually inspected before each use and monthly thereafter with a documented inspection record which includes the date of inspection and an identity of the sling that was inspected.

Please Review The Following for Slings:

A.      Missing or illegible sling identification.

B.      Acid or caustic burns.

C.      Melting or charring of any part of the sling.

D.      Holes, tears, cuts or snags.

E.      Broken or worn stitching in load bearing splices.

F.      Excessive abrasive wear.

G.     Knots in any part of the sling.

H.     Discoloration and brittle or stiff areas on any part of the sling.

I.       Pitted, corroded, cracked, bent, twisted, gouged or broken fittings.

J.      Other conditions that cause doubt as to continued use of a sling.

NOTE: If there are any items that need to be repaired, the sling must be "Tagged Out" until maintenance is completed or discarded.
